New Patient Registration

The doctors welcome new patients who live within our practice area as outlined in the map below.  As it often takes sometime for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire and will be invited for an initial health check with one of our practice nurses. This is an opportunity for us to obtain important medical history before your records arrive.

We are happy to register patients who are staying in the area for a short period e.g. students and temporary residents. You will need to call into the practice to complete a Temporary Resident Registration Form before you make an appointment to see a GP or nurse. Visitors from countries that have a reciprocal health arrangement can be treated on the NHS; if this is not the case a fee for consultation and treatment will be payable.

All patients wishing to register as a permanent or temporary resident will need to provide identification to confirm who they are and where they live.

To confirm who you are, we will need photo id. Please bring one of the following plus your NHS number (not NI number). Your NHS number can be obtained from the doctor’s surgery which you are leaving, or your medical card:-

– Full UK passport

– Full foreign passport & copy of VISA if required

– UK driving licence

– Blue Disabled driver’s pass (must be current)

– European Union member state identity card

– Police warrant card

– Armed Forces ID card

If you have changed your name you must have a supporting document regarding the change of name, e.g. marriage certificate, Decree Absolute or Deed Poll Document.

To confirm where you live, we will need one of the following:

– Driving Licence with current address

– European Union member state identity card (if not used as ID)

– Current Utility Bill (within three months of registration date)

– Local Authority Council Tax Bill (current financial year)

– Council rent book (must be current)

If you have recently moved to the area and have yet to receive any of the above, a confirmation letter from either your solicitor or the council stating your new address.

Please provide us with a photocopy of your documents plus the original for us to view.

You may print off the Registration Form below, fill them out and bring them with you on your first visit to the practice with your identification; please bring original documents as proof of identification. Alternatively, call in and request the forms from one of our reception team.  Please note we require a separate form completing for each patient who wishes to register at the practice.
